VHA’S HEART OF HOME CARE AWARD – CALL FOR 2019 NOMINATIONS

Each year, VHA Home HealthCare recognizes the true heart and soul of home care – the family members and friends caring for people living with chronic disability and disease. Do you know an unpaid caregiver whose caregiving is nothing short of spectacular? Then why not submit a nomination? We are accepting nominations of non-professional* caregivers in Ontario until February 10, 2019.

Award Categories

Drive to thrive
This caregiver shows tenacity in the face of their loved ones diagnosis. Using encouragement and support and determination they have directly brought about positive change to their loved one’s condition and life. These are stories of defying expectations and/or recovery (accidents, stroke, recovery from cancer, long term disability).

Caring and Sharing
This caregiver isn’t just an amazing support for their loved one they are also sharing their knowledge of their journey to help inform and guide others who may face similar challenges (active on social media, online or in person community, fundraising, mentorship, church or community group this person is helping to make a difference to people).

Young Caregiver
The award recognizes the extraordinary efforts of a (under 25 years old) friend or family member who has gone above and beyond the call of duty to provide a loved one who is disabled, elderly, or chronically ill, with the support needed to live at home with more independence.

Caring with Compassion
The award recognizes the extraordinary efforts of a friend or family member who has gone above and beyond the call of duty to provide a loved one who is disabled, elderly, or chronically ill, with the support needed to live at home with more independence.
